Children and Youth with Special Health Care Needs Program

Children & youth served by the CYSHCN program have, or are at risk for an illness or condition for which they need extra health care and support services than their peers in the same age group. These children may have a mild to serious or long lasting physical condition, intellectual or developmental disability and/or behavioral or emotional condition. The CYSHCN program strives to improve the system of care for children & youth from birth to 21 years of age and their families. The program can help families find a health care provider, assist in obtaining health insurance or other programs that can help cover health care costs, refer families to other community services & programs. Families that believe their child may benefit from this program are encouraged to contact JCPHS at 315-786-3730 for more information.
Conditions include but are not limited to ADD or ADHD, allergies, anxiety, arthritis/joint problems, asthma, autism or autism spectrum disorder, behavioral/conduct problem, blood disorders, Cerebral Palsy, Cystic Fibrosis, depression, developmental delay, diabetes, Down syndrome, epilepsy/seizure disorder, genetic or inherited condition, head injury, hearing problem, heart problem, intellectual disability, learning difficulty, migraines, other mental health condition, muscular dystrophy, sickle cell anemia, speech or language disorder, substance use disorder, Tourette syndrome, and vision problem. The program is not limited to these conditions.

Child Find Program: A state funded program that is connected to, and part of Early Intervention. The goal is to improve the identification, location, referral to care and follow-up of infants and toddlers age 0-3 who may be at risk for physical and developmental disabilities and/or delays.
