Customer Welcome Packet

The following forms are part of our Customer Welcome Packet.  Please print out the required forms and bring them with you for your first appointment.  The initial appointment does take about 45 to 60, so we ask that you please arrive no later than 3:30 if you wish to register with us.  If you have a resume please bring that with you as well.  You do not need to register to attend a workshop,  an open interview session or our job fairs.  If you have any questions, please do not hesitate to contact us!
